He gave Birrell authority to pass the signal ahead at danger, at caution speed. D5122 then set off, but McIntosh took his engine to around 40 mph (65 km/h). As result, when the Up DMU became visible to him, he was unable to stop in time and collided with the rear of the Up DMU. Both trainmen were killed instantly.
